Notification of Destruction of Records
NOTIFICATION OF DESTRUCTION OF
SPECIAL EDUCATION STUDENT RECORDS FOR FORMER STUDENT
In accordance with state and federal regulations implementing the Individuals with Disabilities Education Act (IDEA), this notice is to inform you of Jenks Public School’s intent to destroy personally identifiable information related to special education services maintained on students who received special education services under an Individual Education Plan while attending Jenks Public Schools but who have not been enrolled for seven or more years.
Records for students who have not been enrolled for seven or more years will be destroyed beginning November 1, 2021. With proof of identity, the parent/guardian or eligible (adult) student may request a copy of the records in person or by email.
Please be advised that the records may be needed by you for social security benefits or other purposes.
If you wish to maintain this information for your personal records, please contact Student Programs/Special Education at (918) 299-4411 x2272.
Records to be destroyed include:
- Individualized Education Programs (IEPs)
- Evaluation Reports - Test Protocols
- Review of Existing Data Summaries
- All other personally identifiable information within the Special Education file
The reason for the destruction of the above-listed items is that they are no longer needed to provide educational services.
