Name & Address Changes
It is important that Jenks Public School and benefit vendors have your current name and address to avoid lost mail. The procedure is for you to obtain a Name/Address Change Form from your site secretary, and or the Human Resources office to change your personnel records.
State Insurance Plans: Once the Human Resources office receives your change form a copy will be forwarded to the Insurance Office. The insurance office will make the change on-line with the State for your health, dental, vision and/or life insurance.
Flexible Benefit Plans: To change a name or address for the Flexible Benefit plans, send a signed memo to American Fidelity Assurance Company, PO Box 25510, Oklahoma City, OK 73125. You may also change your address by checking the new address box the next time you file a flex reimbursement claim form

Cancer, Income Protection, Accident, Life, GAP and Long-Term Care If you are currently enrolled in these plans, you may call 1-800-662-1113 to notify American Fidelity Assurance Company.
